Gestational diabetes :
Defined as diabetes with first onset or recognition during pregnancy
Physiology :
Pregnancy is a ( diabetogenic state ) with increased insulin resistance and reduced peripheral uptake of glucose ( due to placental hormones with anti-ansulin activity )
In this way , the fetus has a continuous supply of glu.
Risk factors:
Obesity , ethnicity ,family history of type2 diabetes , and previous macrosomia or gestational diabetes .
Maternal complications :
Gestational diabetes poses little risk to the mother
Diabetes occurring during pregnancy increases the risk of maternal pyelonephritis.
Some of women with this disorder develop 2 diabetes .
some of women develop gestational diabetes in a subsequent pregnancy .
fetal complications :
fetuses are exposed to a high concentrations of glu.
Fetal macrosomia ( associated with an increased risk of cesarean delivery and birth injury )
Treatment :
Treatment is the diet in the first instance, but most patients require insulin cover during the pregnancy (insulin doesn't cross the placenta^-^) (many oral agents are avoided because of the potential risk to the fetus@-@)
Screen test :
Between 24th -28th of pregnancy
